Left sidebar contains:

  • Dashboard - Home screen with overview
  • Patients - Patient list and management
  • Intake Forms - All form submissions
  • Users (Admin only) - User management
  • System (Admin only) - Settings and audit logs

Dashboard Home

Shows at a glance:

  • Recent Submissions - Latest form submissions
  • Pending Reviews - Forms awaiting staff review
  • Active Patients - Patients with assigned forms
  • Quick Actions - Shortcuts to common tasks

Patients Section

Patient List

  • All patients in your clinic
  • Search by name or DOB
  • Sort by name, date added, or recent activity
  • Click patient to see details

  • Patient information
  • Assigned forms
  • Completed submissions
  • Assignment history

Actions

  • Add New Patient
  • Edit Patient

Submissions List

  • All form submissions
