Start new intakes and review form submissions.

Access

Admin Portal → Intake (sidebar or home card)

Two options presented as cards:

Start Intake

Begin new TMS patient intake.

Opens the full medical intake questionnaire.

Use this when staff is helping patient fill out form in-person.

Form Submissions

View and review all completed intake forms.

See list of all submissions with:

  • Patient name
  • Submission date
  • Form type
  • Status

Start Intake Workflow

  1. Click “Start Intake” card
  2. Fill out form sections:
    • Demographics & Safety
    • Medical History
    • Providers & Treatment Goals
  3. Form auto-saves progress
  4. Submit when complete
  5. Confirmation message shown

Same form that patients use. Staff can walk patient through it or fill it out with patient.

Form Submissions View

Click “Form Submissions” card to see all completed intakes.

Table columns:

  • Patient name
  • Form type (e.g., “TMS Intake”)
  • Submission date/time
  • Status (submitted, reviewed, approved)

Actions per submission:

  • Click row to view full submission details
  • Generate AI summaries (if LM Studio running)
  • Export to PDF
  • Change status

Search and filter:

  • Search by patient name
  • Filter by date range
  • Filter by status
  • Sort by any column

Viewing a Submission

Click any submission to see:

Patient Info section:

  • Name, DOB
  • Contact information
  • Insurance details

Form Data section:

  • All questions and answers
  • Organized by section
  • Readable format

AI Summaries section (if generated):

  • Clinician summary
  • Patient summary
  • PA summary
  • Edit any summary if needed

Actions:

  • Generate AI summaries
  • Export to PDF
  • Print
  • Change status (submitted → reviewed → approved)
  • Add notes

Back Navigation

Every subpage has “Back” button in top left.

Returns to Intake Hub card menu.

From there, sidebar or home button to navigate elsewhere.
