Managing Patients

Add a Patient

Dashboard → Patients → Add New

Required fields:

  • First name
  • Last name
  • Date of birth

Optional fields:

  • Email (for sending forms)
  • Phone (for SMS links)
  • Medical record number

View Patient Details

Click patient name to see:

  • All assigned forms
  • Completed submissions
  • Patient information
  • Assignment history

Search Patients

Use the search box at the top of the patient list.

Searches:

  • First name
  • Last name
  • Date of birth
  • Medical record number

Patient Data Security

All patient data is encrypted (PHI).

Every access is logged to audit logs with:

  • Who accessed the data
  • When
  • What action they took
  • Their IP address

Only users in the same clinic can see a patient’s data (enforced by row-level security).

Edit Patient Information

  1. Click patient name
  2. Click “Edit” button
  3. Update fields
  4. Save

Changes are logged in audit trail.

Delete Patient

Warning: This permanently deletes all patient data including form submissions.

Only administrators can delete patients.

The action is logged and cannot be undone.
