Actions

Begin Intake

Click “Begin Intake” to start a new TMS patient intake form.

Takes you to the full medical intake questionnaire with:

  1. Demographics & Safety
  2. Medical History
  3. Providers & Treatment Goals

View Submissions

If you have previous submissions, click “View my submissions” to see them.

Shows number of submissions in parentheses.

Go to Admin

If you’re an admin, click “Admin” in the top right to access the admin portal.

Log Out

Click the logout icon (top right) to sign out.
