View Submissions

Dashboard → Intake Forms

The list shows:

  • Patient name
  • Form type
  • Status (submitted, reviewed, approved)
  • Date submitted
  • Assigned to

Review a Submission

Click any submission to see:

  • Patient information
  • Form responses (all questions and answers)
  • AI-generated summaries (if generated)
  • Submission timestamp
  • Who submitted it

AI Summaries

Click “Generate AI Summaries” button to create three summaries:

  1. Clinician Summary - Key clinical decision points for staff
  2. Patient Summary - Plain-language explanation for patient
  3. PA Summary - Medical necessity letter for insurance

Requires LM Studio running on localhost:1234.

AI summaries are optional—you can review submissions without them.

Change Status

Use the status dropdown to update:

  • Submitted - Initial state
  • Reviewing - Staff is actively reviewing
  • Approved - Intake approved for scheduling
  • Needs Revision - Requires more information
  • Rejected - Intake rejected (note reason)

Status changes are logged in audit trail.

Add Notes

Click “Add Note” to attach staff notes to the submission.

Notes are visible to all staff but not to patients.

Export Data

Click “Export” to download submission as:

  • PDF (formatted for printing)
  • JSON (raw data for integration)
  • CSV (spreadsheet format)

Click “Print” for a printer-friendly view.

Includes all form data but excludes internal notes.
