Can’t Log In

Problem: Login fails or redirects back to login page

Solutions:

• Check email/password spelling
• Clear browser cache and cookies
• Try incognito/private mode
• Ask admin to check your account status
• Try different browser

Patient Portal Link Broken

Problem: Patient clicks link and gets 404 error

Solutions:

• Check form assignment is still active
• Link expires after form is submitted
• Regenerate link from patient page
• Verify link was copied completely (no truncation)

AI Summaries Not Working

Problem: “Generate AI Summaries” button fails

Solutions:

• Check LM Studio is running on localhost:1234
• Enable CORS in LM Studio server settings
• Check browser console for error details
• Verify model is loaded in LM Studio
• Remember: AI summaries are optional—you can skip this feature

Submissions Not Showing

Problem: Patient submitted form but staff doesn’t see it

Solutions:

• Refresh the page (F5)
• Check if you’re viewing the correct clinic
• Verify patient’s form assignment status
• Look in audit logs for submission event
• Check submission_status in database

Slow Performance

Problem: Pages load slowly or time out

Solutions:

• Check your internet connection
• Cloudflare might be experiencing issues
• Try different browser
• Check browser dev tools for errors
• Contact dev team if problem persists

Export/Print Not Working

Problem: Export or print buttons don’t work

Solutions:

• Check browser popup blocker
• Enable JavaScript
• Try different format (PDF vs JSON)
• Check browser console for errors

Missing Audit Logs

Problem: Can’t see audit logs (admin only feature)

Solutions:

• Verify you have admin role
• Logs may take a few seconds to appear
• Check filter settings
• Contact dev team if logs are missing
